* The Committee encourages the applicant to submit a personal statement along with the
application form to help us fully evaluate the circumstances of this request and the opportunity it provides the applicant.
** This application will be reviewed solely by the three members of the Special Opportunity Fund Committee. All applications and records of applications will be confidentially maintained.
*** Please submit applications to the church office or via email to a committee member by the following review dates: December 31st and May 31st. Applications will be considered on an emergency basis. Applications must be submitted by the applicant, not a third party. The Committee will determine the amount of the grant and notify the applicant within a month of receiving the application. The grant award may be different  than the amount requested based on the availability of funds at the time the request is made.
